The words "transported for", "tried at" and "arrived per ship" on the form have been crossed out. On the left hand corner of the form are printed the words "Commt V.D.Land 1000 10-53" The form is double sided parchment with handwritten entries and it bear the name of William Sewell who arrived in Van Dieman's land on the Siam.
